Picoprep 2 Sachet

Pico-Prep is a systemic oral purgative indicated for bowel evacuation and colon cleansing prior to diagnostic colonoscopy, surgical bowel resections, or radiological abdominal examinations. It combines two pharmacologically distinct and synergistic bowel-cleansing mechanisms: Sodium Picosulfate (a local stimulant laxative) and Magnesium Citrate (an osmotic laxative formed in situ when magnesium oxide and citric acid react in aqueous reconstitution). Sodium picosulfate is cleaved by bacterial sulfatases within the colonic lumen to produce bis-(p-hydroxyphenyl)-pyridyl-2-methane (BHPM), directly stimulating peristaltic motility in the colonic wall. Concurrently, magnesium citrate retains high volumes of water within the colonic lumen via osmotic force, diluting fecal contents and inducing rapid mechanical evacuation. It is supplied as a powder for oral solution with an orange flavor, packaged in a box containing 2 unit-dose sachets.

How to Use

• Reconstitution: Dissolve the contents of 1 sachet in a glass of cold water (approx. 150 ml). Stir for 2 to 3 minutes until the powder completely dissolves; the solution may become warm due to the acid-base reaction—allow it to cool before drinking.

• Split-Dose Regimen (Standard):

  • First Sachet: Ingested the afternoon or evening (e.g., 5:00 PM to 8:00 PM) before the scheduled morning procedure, followed by drinking at least 5 glasses of clear fluids (approx. 250 ml each, total 1.25 L) over the next few hours.

  • Second Sachet: Ingested the following morning approximately 4 to 6 hours prior to the procedure (or the preceding late evening depending on hospital protocol), followed by drinking at least 3 glasses of clear fluids (approx. 750 ml) up until 2 hours before the examination.

• Mandatory Hydration Protocol: Consuming the required clear liquids (water, clear broth, diluted apple juice, electrolyte solutions) is essential to prevent dehydration and hypovolemia.

• Contraindicated in cases of congestive heart failure, severe renal insufficiency, gastrointestinal obstruction, ileus, toxic megacolon, gastric retention, or active bowel perforation.
