DESCRIPTION

Diamicron MR 60 mg is an oral modified-release hypoglycemic medication formulated with Gliclazide (60 mg), a second-generation sulfonylurea with an endocyclic N-containing azabicyclo-octyl heterocyclic ring. Gliclazide acts primarily as a pancreatic secretagogue by binding selectively with high affinity to the sulfonylurea receptor-1 (SUR1) subunits of ATP-sensitive potassium channels (K-ATP) located on the plasma membrane of pancreatic beta-cells. This binding closes the K-ATP channels, inducing cell membrane depolarization, opening voltage-dependent L-type calcium channels, and triggering regulated calcium influx. The elevation of intracellular free calcium promotes the exocytosis of insulin-containing secretory granules, partially restoring the physiological early first-phase insulin secretion peak and augmenting the second phase in response to ambient glucose concentrations. Uniquely among sulfonylureas, gliclazide exhibits independent hemovascular protective properties, attenuating microvascular thrombosis by inhibiting platelet aggregation and adhesion and restoring physiological fibrinolytic activity via tissue-type plasminogen activator (t-PA) upregulation. Formulated in a hydrophilic hypromellose matrix, each breakable modified-release tablet ensures continuous, linear drug release matching the 24-hour circadian glycemic profile, packaged in blister strips containing 30 breakable modified-release tablets.

Key Benefits

  • Glycemic Control: Enhances endogenous insulin release, significantly lowering fasting blood glucose (FBG), postprandial glucose (PPG), and glycosylated hemoglobin (HbA1c).

  • Restoration of First-Phase Insulin Secretion: Improves the rapid initial insulin secretory surge typically blunted in early type 2 diabetes, mitigating postprandial glycemic excursions.

  • Hemovascular & Endothelial Protection: Counteracts microvascular complications by reducing platelet hyper-reactivity, lowering platelet-activating markers, and neutralizing free radicals via its azabicyclo-octyl moiety.

  • Lower Hypoglycemia Risk: Exhibits reversible, selective pancreatic SUR1 binding without non-specific interaction with cardiac (SUR2A) or vascular smooth muscle (SUR2B) channels, carrying a lower incidence of severe hypoglycemia compared to glibenclamide.

  • 24-Hour Circadian Coverage: Hydrophilic matrix guarantees sustained, steady systemic absorption with a single morning dose, aligning with the daily pharmacokinetic requirement.

Key Ingredients (per tablet)

  • Gliclazide (60 mg): Active second-generation sulfonylurea oral antidiabetic agent.

  • Modified-Release Matrix & Excipients: Hypromellose (hydroxypropyl methylcellulose 100 and 4000), calcium hydrogen phosphate dihydrate, maltodextrin, colloidal anhydrous silica, and magnesium stearate.

Key Features

  • Product Type: Oral Hypoglycemic Agent / Modified-Release Sulfonylurea (Prescription Antidiabetic Medication)

  • Indications: Non-insulin-dependent Type 2 Diabetes Mellitus (T2DM) in adults when dietary measures, physical exercise, and weight loss alone are insufficient

  • Hydrophilic hypromellose sustained-release matrix

  • Breakable oblong tablet with score line (allows splitting into two equal 30 mg halves)

  • Pack size: 30 modified-release scored tablets

How to Use

  • Take orally as a single daily dose at breakfast time (or with the morning first meal).

  • Standard maintenance dosing ranges from 30 mg to 120 mg once daily (half a tablet to two tablets once daily), titrated gradually based on individual blood glucose and HbA1c targets.

  • Swallow the tablet (or halved tablet) whole with a full glass of water; do not chew or crush the tablet matrix, as this destroys the modified-release delivery mechanism.

  • Regular daily meals containing carbohydrates must be consumed following ingestion to prevent hypoglycemia.

  • Contraindicated in Type 1 diabetes, diabetic ketoacidosis, diabetic pre-coma/coma, severe renal or hepatic insufficiency, concomitant treatment with miconazole, and during pregnancy or lactation.
